Partner with Us

Rehab America typically works on-site in skilled nursing facilities and post-acute hospitals to provide therapeutic interventions to improve health outcomes for those recovering from illness or injury.

We specialize in creating partnerships with providers that support their mission and help them improve patient care.

Our partners in Tennessee and Alabama include American Health Communities (AHC) with 29 skilled nursing facilities, and Unity Psychiatric Care with five hospitals.

We would appreciate the opportunity to discuss partnering with your facility or practice. Just let us know what works for you. Full-service contract therapy services, cost-plus agreements or management contracts are all options.

We Excel at Partnering with Skilled Nursing Facilities

For over two decades, Rehab America has specialized in providing care in skilled nursing facilities. Our therapists have worked side-by-side with nursing home caregivers to deliver individualized care to thousands of residents. This experience attending to those living in nursing homes gives us a unique insight into the holistic needs of aging patients.

Being integrated into the nursing home’s care teams means we are present for care decisions involving the resident and their family. We can work collaboratively with the nursing home to adapt the environment to aid recovery and help individuals be as independent as possible. It’s important to us that every patient has the chance to reach their highest potential. This includes all aspects of life as we walk next to our patients on their path to healing.

Payors

Rehab America provides skilled care and outpatient services through our partners who in turn establish reimbursement with the payors. We are committed to working with a variety of payors and helping you are fully reimbursed for all services provided.

Some payors we frequently work with are Medicare Plans, Managed Care, Institutional Special Needs Plans (ISNPs).
